Requirements:
- Required Certifications: BLS, MSW
- Required Skills: Biopsychosocial assessment, Collect and analyze patient information, Conduct initial evaluation, Coordinates with internal and external team members, Discharge Planning, Documents pertinent information to facilitate care coordination, Drug/alcohol use and treatment options, Facilitate discharge and contingency planning, Guardianship, Adult protective services, Case management, Child protective services, Collaborative care, Coordinate services to home care and equipment following discharge, Develop plan of care to meet patient/caregiver needs, Discharge Planning, Discharge Summary, Documentation according to plan of care, Documentation reflects understanding of coverage criteria, Documents patient response, Emergency detention, Facilitate social and psychological well being of nursing home residents, HMOs, Knowledge of cultural issues, Management/lead experience, Medicaid, Medical Transportation, Medicare, NASW Code of Ethics/Standards of Home Health Care, Participates in care conference activities, Plan of Care, Power of Attorney, Refers to rehab and long term care facilities, Review of Health history, Social Security, Plan of Care, Refers to other disciplines/resources
